It is the primary tributary of the Great Liakhvi River, which it joins near the city of Gori. Originating on the Keli Volcanic Highland, a remote and high-altitude plateau, the Patara Liakhvi flows south for over 60 kilometers, carving its own significant gorge parallel to the main Liakhvi valley. The river is a vital waterway in a region of immense historical importance and modern political complexity.

The upper valley of the Patara Liakhvi is a remote, beautiful mountain landscape, while its lower course flows through the fertile plains of Shida Kartli, a major agricultural zone.
